The Rossies led by seven points to a goal and two points at the interval with the sides level at a goal and eleven points apiece at full-time.
It was the Ulster champions who held the advantage at half-time in extra-time, by a solitary point.
However, the primrose and blue prevailed, registering a goal and a point to their opponent's solitary effort in the second half of extra time to book their spot in the decider.
They will face the winners of the other semi-final between Laois and Tipperary.
