Portiuncula University Hospital reported 69 clinical incidents in its maternity unit over January and February of this year.
A clinical incident is any event that could have or did result in harm to a patient.
It comes as a recent infant death at the Galway hospital is now under external review, bringing the number of ongoing investigations to 10.
These include seven cases where babies were diagnosed with hypoxic ischemic encephalopathy (HIE) and two stillbirths.
A new management team was appointed by the HSE in January to oversee care at the unit.
A 2018 report also identified significant care failings at the unit in previous years.
Health Minister Jennifer Carroll MacNeill says patient safety must be the top priority at Portiuncula and across all maternity hospitals.
