An inquest in 2024 heard some advice from Beaumont Hospital was not followed at Mullingar Hospital.

The Health Minister says failures occurred in the treatment of a county Longford woman who died in HSE care.

25-year-old Bryonny Sainsbury, from Newtownforbes, died after being crushed by a horse in 2021.

An inquest in 2024 heard some advice from Beaumont Hospital was not followed at Mullingar hospital, and an earlier transfer to Dublin might have saved her.

Health Minister Jennifer Carroll MacNeill and HSE CEO Bernard Gloster issued an apology to her family today, saying failures were evident:
