The health watchdog has issued a damning report on two county Roscommon centres for older people.
The two premises, located in the north and west of the county were found to be non-compliant in a number of areas.
HIQA's unannounced inspections took place in May and June of this year, the details of which have been published today.
Abbey Haven Care Centre and Nursing Home is a purpose-built facility which can accommodate a maximum of 63 residents.
Bedroom accommodation consists of single and twin rooms, all with full ensuite facilities.
Several areas, including bedrooms, laundry and storage rooms were visibly unclean.
Inspectors found unsafe storage of flammable items and insufficient fire door maintenance.
While dining arrangements were also found to be a limited choice with some residents not given full access to the dining room experience.
Aras Mhathair Phoil provides 24-hour nursing care to 24 male and female residents.
Accommodation consists of 12 single and six twin bedrooms.
The inspection found that management structures and oversight mechanisms remain weak. Audits are conducted, but actions were not consistently implemented.
A Clinical Nurse Manager post—previously filled—had been vacant since May 2025 which undermined leadership and continuity of care.
